Overview

Heart disease risk is shaped by several factors working together, blood pressure, cholesterol, weight, activity level, and family history. A primary care provider who sees the whole picture over time can help build a plan focused on all of it, not just one number at a visit.

How ongoing primary care can help

Your provider will assess your combined risk factors and help coordinate a plan that may include medication, referrals to cardiology when appropriate, and specific, trackable lifestyle goals.

When to seek emergency careChest pain or pressure, pain radiating to the arm or jaw, or sudden shortness of breath are medical emergencies, call 911.

Frequently asked questions

Can heart disease risk management really be managed over telehealth?
In many cases, yes. A primary care provider can evaluate your symptoms and history over video, review any recent labs, and help adjust your treatment plan when appropriate. For some chronic conditions, especially if they are uncontrolled, our providers may need to collect/complete vital signs, physical exams, lab tests, or imaging which requires an in person visit. Our providers will help you take the next step.
Do I need to keep seeing the same provider?
Continuity helps, as seeing the same provider over time means they already know your history and current plan, which usually means faster, more informed visits.
What if I need to be seen in person?
If your provider determines your condition needs an in-person exam, testing, or treatment they can't provide virtually, they'll let you know and help direct you to the right next step.
